Children's Occupational Performance Questionnaire (COP-Q)

The Children’s Occupational Performance Questionnaire (COP–Q) is a measure of occupational performance designed for use by occupational therapy practitioners and other rehabilitation and educational professionals who are interested in gathering information about a child’s ability to perform their meaningful daily activities. Designed to be completed primarily by caregivers, it can also be completed as self-report by children 16 years of age or older. 


A norm-referenced, standardized measure developed and validated over more than 10 years of research, the COP–Q measures a child’s capacity to perform expected, desired, or required self-help, play and leisure, social, school-related, and other daily living tasks and activities and the extent to which it differs from most children, suggesting a developmental delay or impairment. This information can be used to assist in determining eligibility for disability-related services, planning intervention or educational planning, or evaluating functional change in performance over time or following intervention.


As a comprehensive measure of occupational performance, the COP–Q addresses all areas of occupation as defined in the Occupational Therapy Practice Framework (and is quick and easy to use. The instrument is designed to be used with children with a wide variety of conditions. Both norm- and criterion-referenced scoring also evaluate levels of satisfaction by the caregiver or, in the case of self-report for older adolescents, the respondent.
